CCM is for management of two or more chronic conditions, PCM is for management of one chronic condition.
PCM is for a condition that lasts 3 months or longer, vs. CCM is for a condition that lasts 12 months or longer.
Per your care focus, KangarooHealth helps you qualify eligibility for your patients. General guidelines include:
• Patients who qualify for PCM have been managing one chronic condition for at least 3 months. Patients who qualify for CCM have been managing two or more chronic conditions for at least 12 months.
• CCM/PCM program members should be established patients who you have had a prior evaluation and management service with (this can occur in person or over telehealth).
• Patients may not be currently enrolled in another CCM/PCM program.
• Nursing home patients do qualify for CCM/PCM.
• Many patients have $0 out of pocket expenses.
• Depending on insurance plans, some patients may have minimum financial responsibility such as co-pays.
• KangarooHealth will contact the patient with an estimate of their copay for approval prior to final enrollment in the program.
Patients can only be enrolled in one CCM/PCM program by a provider at any given time. During patient enrollment, KangarooHealth confirms with patients that they are not already enrolled in a CCM/PCM program.
Patients enrolled in CCM/PCM can also enroll in either a RPM or RTM program through KangarooHealth, which could help improve outcomes for patients with comorbidities and are billed separately through different CPT codes.
PCM and CCM cannot be applied to the same patient in the same month.
Very! Our expert clinical monitoring staff members become a trusted coach. We also communicate with patients using the channel they prefer, including phone, SMS, in-app messaging and/or telehealth.
There is no provider obligation. KangarooHealth also sends a monthly report directly to your EMR.

Providers are required to submit the reimbursement report. Most of our customers choose to handle billing in reference to monthly summary reports provided by KangarooHealth. We do have full revenue cycle support available on request.

No, CCM/PCM is not encounter based. When KangarooHealth is providing the patient monitoring and engagement there is no requirement that staff from your office be in touch with the patient each month in order to bill.
KangarooHealth can have your program up and running within two weeks. We have a streamlined onboarding process that involves just 2-3 hours of education time required by your staff.
Referring patients takes a few clicks if we’ve completed a simple integration with your EMR. If your staff is using the KangarooHealth platform for referrals it typically takes one minute.
KangarooHealth does the verification of insurance and patients can usually begin the program within 24-48 hours after referral.
