Your Questions, Answered: Everything You Need to Know About Chronic Care Management
- Patients who qualify for CCM have been managing two or more chronic conditions for at least 12 months.
- CCM program members should be established patients whom you have had a prior evaluation and management service (this can occur in person or over telehealth).
- Patients may not be currently enrolled in another CCM program.
- Nursing home patients do qualify for CCM.
